Forms and Guidelines

Students please login to your HSUHK email account (hsu.edu.hk) to download and fill-in the forms.

Before the trip

  1. Fill out the Pre-trip approval form (MS Form) and upload all required items.

After the trip

  1. Download the Reflective Report (Word) template and fill it out.
  2. Email your completed report to your Personal Tutor (PT) for endorsement.
  3. Once approved, upload your report, the PT’s endorsement email, and other required items to the Reflective Report (MS Form).
